Great Lakes Periodontics strives to create an environment that is collaborative between our staff, your general dentist, and you. Your treatment plan is developed with full awareness of the details of you and your needs, and regular communication about your progress.
To help us develop a treatment plan that best aligns with your goals, please be sure to notify us of any information relating to your health history, changes in symptoms, or any other information since your last visit.
All fees are payable at the time of service. We accept cash, personal single party checks, credit cards. Please call our office at 616.940.2854 to discuss your payment options.
For your convenience, if you have dental insurance, we will provide a printed claim form so that you may file the claim with your insurance company. Full payment is expected at the time of service. Your insurance company will send any reimbursement directly to you. We are always happy to assist in determining your insurance benefits in relation to your treatment plan as requested. Please note that our treatment recommendations and fees are based on your needs, not what your insurance may or may not provide for.
Other Payment Options
In some cases, our services usually qualify for reimbursement through your Flexible Spending Account or Healthcare Savings Account as provided by your employer.
We are also partnered with Care Credit to help manage the financial aspect of your treatment.
